Direct Care Provider-Team Lead

Mentor and motivate. In this role, you’ll combine direct care with leadership — guiding your fellow caregivers, ensuring quality service, and keeping our mission strong in every shift and setting.

Position Overview:
Unique Caring Opportunities, Inc. is seeking a highly motivated and compassionate Team Leader to support and oversee our direct care team in delivering exceptional care to clients. This role combines hands-on care with leadership responsibilities, ensuring the highest standards of service while fostering a positive and productive work environment.

Key Responsibilities:
Direct Care:

● Provide personal care and assist clients with daily living activities.
● Build and maintain positive relationships with clients and their families.
● Monitor client health and well-being, reporting any changes to the manager.


Leadership and Supervision:

● Serve as a role model and mentor for direct care staff, offering guidance and support.
● Fill shifts or roles as needed to maintain consistent client care.
● Oversee medication administration processes to ensure accuracy and compliance with guidelines.
● Conduct audits of care practices and documentation to uphold quality and regulatory standards.
● Assist in the development and implementation of individualized care plans.
● Facilitate clear and effective communication between direct care staff and management.
● Address staff or client concerns promptly and professionally, fostering resolution and team cohesion.


Facility and Safety Management:

● Ensure the cleanliness, functionality, and safety of group home facilities.
● Oversee and schedule appointments to address client health and safety needs.
● Train staff on care procedures, compliance issues, and areas needing improvement.
● Support admissions and discharges as required.


Qualifications:
Required:

● Certified Direct Care Provider or equivalent certification.
● Proven experience in a direct care role, with leadership or supervisory responsibilities.
● Comprehensive understanding of medication administration and compliance standards.
● Strong communication, organizational, and interpersonal skills.
● Flexibility to work varied hours, including filling in shifts as necessary.
● Ability to lead and inspire a team while ensuring accountability.


Preferred:

● Experience in conducting audits and implementing quality improvement strategies.
● Training in CPR and First Aid.
● Familiarity with healthcare regulations and best practices for group home environments.


Physical Demands:
● Ability to perform physical tasks such as lifting, bending, and standing for prolonged periods.


Work Environment:
● Operates in a professional healthcare and residential setting, requiring adherence to safety and care standards.
